Yes. This is true for all IRS Tax forms.

Although the totals are shown on the CU*BASE member tax inquiry as separate totals by account number, the forms combine these totals on one form. There is an "Account Number" box on the form that lists up to 2 account numbers under the given SSN.

This is important to note if a "corrected" form needs to be submitted.
